And enter your email address working email address don't put the email that you don't have access to it, confirm the email address 

Then select where you're using WhatsApp with if its on web, Android, iPhone or desktop. Then select the one you used

Then fill the message box with your complaint and tell them what you think they banned your mobile phone number if you're using third-party app like GB WhatsApp and FM WhatsApp then apologize and send the message you will get a feedback and they will unbanned your WhatsApp Number.
